Death Claim
1. Duly completed claim form
2. Submit original Policy document, together with: 2a. Original copies of the following substantiating documents (the documents will be returned to you after sighting): 2ai. Medical Certificate of Cause of Death, 2aii. Death Certificate, 2aiii. Burial Certificate, 2aiv. Coroner's report (if death is by mysterious circumstances), 2av. Police Report (if death is by road traffic accident).
3. Complete the following forms: 3a. Proof of claim on death (to be completed by the deceased Personal Medical Attendant) , 3b. Declaration to establish identity in case of deceased policyholder (to be completed by someone other than a relative or the Medical Attendant or person interested in the assurance), 3c. Proof of claim on death (to be completed by the claimant).
Termination
Applicable under GT Life Savings Plus, Assur Life Plus (ALIP), Assur Whole Life (after 2 years)
1. Complete a termination form or send a letter of intention to disengage from the scheme, 2. Submit original policy document.
Part Liquidation
Applicable under GT Life Savings Plus only.
1. Complete a withdrawal form or send a letter of intention to part-liquidate.
Loan
Applicable under GT Life Savings Plus, Assur Life Plus (ALIP), Assur Whole Life (after 2 years).
1. Complete a loan form or send a letter of intention to take a loan (loan amount subject to a maximum of 70% of contribution/surrender value), 2.Submit original policy document.
Group Business:
Send a claim's notification letter, together with:
1. Original copies of the following substantiating documents (the documents will be returned to you after sighting): 1a. Medical Certificate of Cause of Death; 1b. Death Certificate; 1c. Burial Certificate; 1d. Coroner's report (if death is by mysterious circumstances); 1e. Police Report (if death is by road traffic accident).
